Abstract
Chronic fatigue syndrome, also termed myalgic encephalomyelitis, is a disabling condition that remains underdiagnosed in primary care. We report a 53-year-old woman who developed persistent exhaustion, cognitive impairment, and social withdrawal after postoperative sepsis, leading to the diagnosis of chronic fatigue syndrome. The condition is characterized by post-exertional worsening of symptoms, unrefreshing sleep, and cognitive dysfunction, with multifactorial pathophysiology involving immune, neurological, metabolic, and endocrine pathways. No curative therapy exists; management relies on multidisciplinary care and energy management strategies. Greater awareness in family medicine is essential for timely diagnosis, advocacy, and social and professional reintegration.
